Summer Camp Participation Agreement

& Full Liability Waiver

This Agreement is entered into by and between Art Studio by Toys Natural (the "Studio") and the undersigned Parent or Legal Guardian ("Parent") of the minor child listed below ("Child") for participation in the Studio’s Summer Camp Program.

1. Program Information

Meals: Please note that the camp does not provide breakfast or lunch. However, light snacks will be available to all participants during the day.

• Hours: 9:00 AM – 1:00 PM

• Location: 145 Dillon Ave, Suite H/G, Campbell, CA 95008

2. Tuition & Payment Policy

• Full payment is due upon registration.

• All payments are final and non-refundable.

• No credits or refunds will be issued for absences, illness, scheduling conflicts, or early withdrawal.

• The Studio reserves the right to deny participation if payment is not received in full.

3. Health, Allergies, and Special Needs

• The Parent affirms that the Child is in good physical and mental health.

• The Parent is required to notify the Studio in writing of any allergies, medical conditions, or special needs prior to the start of camp.

• The Studio is not responsible for adverse health effects due to undisclosed medical conditions or allergies.

4. Supervision and Behavior

• The Studio is not responsible for supervision outside of official camp hours.

• Disruptive, unsafe, or disrespectful behavior may result in dismissal from camp without refund.

• Children must follow the Studio’s rules and instructions for a safe and respectful environment.

5. Pick-Up Policy & Late Fees

• Children may only be picked up by their parent, legal guardian, or an authorized person listed below.

• A late pick-up fee of $50 will be charged for every 20 minutes past the scheduled pick-up time.

• The Studio may request valid photo ID upon pick-up.

6. Responsibility for Personal Items

• The Studio is not liable for lost, stolen, or damaged personal belongings.

• Children are advised not to bring valuables to camp.

7. Medical Authorization

• In the event of an emergency, the Studio may seek appropriate medical care if the Parent cannot be reached.

• The Parent agrees to be responsible for all related medical expenses.

8. Insurance

• The Parent confirms that the Child is covered by health insurance and assumes full responsibility for any medical care expenses arising from participation.

9. Off-Site Activities

• The Parent authorizes the Child to participate in occasional off-site activities (e.g., park walks), under staff supervision.

10. Media Release

• Unless notified in writing, the Studio may use photos or videos of the Child for marketing purposes, including social media, website, and print materials.

11. Attendance & Schedule Policy

• The Parent understands and agrees that camp attendance is limited to the specific days and times selected at the time of registration.

• Participants may not attend on days outside of their assigned schedule.

• Any changes to attendance must be requested in writing and approved by the Studio in advance.

• The Studio reserves the right to refuse entry to any child arriving on an unscheduled day.

12. Acknowledgment & Waiver of Liability

• The Parent or Legal Guardian fully acknowledges and accepts that participation in the Summer Camp activities involves inherent risks, including but not limited to minor injuries (such as cuts, bruises), allergic reactions, accidents during supervised activities, or unforeseen medical emergencies.

• By signing this agreement, the Parent or Legal Guardian voluntarily assumes all such risks and irrevocably waives any and all claims of liability against the Studio, its owners, employees, contractors, volunteers, and affiliates for any injury, illness, loss, damage, or expense that may arise from the Child’s participation in the Summer Camp.

• This includes, but is not limited to, incidents occurring during creative activities, physical games, off-site walks, or during drop-off and pick-up.

• The Studio will exercise reasonable care and supervision; however, the Parent agrees that they are solely responsible for any medical costs or consequences that may result from participation.

• The only exception to this waiver shall be in cases of gross negligence or intentional misconduct by the Studio or its representatives.
